Primary Transition
Greenfield Academy has a comprehensive transition programme for KS2-KS3. We understand and know how important a successful transition is to our students with additional needs.
Greenfield works collaboratively with local feeder primary schools to support students from Year 5, through to arrival in Year 7. As an Academy, we provide additional transition support for students who we feel would benefit from enhanced transition. To support the transition process, we have a designated transition worker, Miss Thurley who works closely with our feeder primaries. Throughout the academic year, Miss Thurley organises regular primary school visits and visits to Greenfield for identified students.
We also run a summer school during the six weeks holiday for Year 6 students, giving them the chance to experience sessions/activities as well as to meet other students who will be joining the school in Year 7. **currently limitations associated with the new build means that plans are fluid and will be confirmed nearer the time.
Students with additional needs will be provided with enhanced transition that supports their specific needs. Individual transition plans are put in place where necessary to support a smooth transition to secondary school.
All students are invited to attend our Induction Day which allows students to experience sample lessons and fun activities, designed to build self-confidence and familiarise themselves with the academy.
We value the importance of including parents and carers as much as possible in their transition to Greenfield Academy. Parents/carers are invited into school in the summer term of Year 6 for our Induction Evening. This gives parents/carers a further opportunity to look around the Academy, meet staff and discuss any concerns that they may have before their child starts in Year 7.
KS4
All SEND children are given priority in careers guidance support. We also provide enhanced transition for KS4 students moving onto their Post 16 choice.
If your child has an EHC Plan (EHCP), they will meet with a Post 16 SEND Caseworker from the Local Authority to create a plan for post 16 education during the review process in year 11. A Durham Works Advisor will also provide support for post 16 choices and preparation for moving onto post 16 education and/or training.
If your child does not have an EHCP, the plan will be created with parents/carers, the pastoral team and the SEND team. A Durham Works Advisor may also provide support for post 16 choices and preparation for moving onto post 16 education and/or training.
From / to another school:
All students arriving at other times in their school career undertake some early baseline assessments. Their support needs are also identified following liaison with their previous school as well as parents/carers. It is important that any additional needs are shared by the parents/carers in the first instance with Greenfield. Where a student transfers out from Greenfield to another secondary school we will support that school in its transition process.
